The vial comes with a blank medical information form for your home and wallet, a Vial of L.I.F.E. sticker and a magnet.  You complete the medical information form within the vial, including:  important phone numbers, preferred hospitals, medical conditions, medications, allergies, power of attorney and more.  Place the vial containing the form on the top shelf of your refrigerator, the magnet on the front of the fridge, and the sticker on your front door, or an adjacent window.  Why?  Emergency medical responders are trained to look for Vial of L.I.F.E. magnets and stickers.  The Vial of L.I.F.E. program will allow you to voice your wishes when unable to speak and give immediate and valuable information to emergency responders, expediting your course of treatment.  In case of fire, your refrigerator acts as a safe, protecting your valuable medical information.
